Americ Transit Services offers safe, reliable wheelchair transportation services for healthcare facilities in the counties of Dekalb, Fulton and Gwinnett counties in the Atlanta metropolitan area. We recognize the need to have a service that can provide on time accessible transportation for medical appointments, hospital discharges and urgent treatment or procedures.

How do you get started?

Step One:

Please proceed to fill out the Billing Authorization Form, providing us with the facility’s primary contact name, your billing address, and the necessary signatures from authorized staff members who are eligible to request private pay transportation. Upon submission of the completed form, you will receive an email confirmation confirming receipt and acceptance of the document.

Step Two:

You can submit a trip request through the website by selecting the Transportation Request Form. You will receive the confirmation by email within 24 hours.
  • Transportation Request Form.
  • A Transportation Request Form (online) should be submitted 48 hours before the scheduled appointment or the
  • Same day requests (we will check availability) should be called in to the office at (877) 287-6368. Please note – there may be an additional fee for same day trips.

Please proceed to fill out the billing authorization form, and your facility will be registered in our system. You can begin scheduling trips when you receive your confirmation email verifying that we have all your information.  It is that easy!

Our drivers will provide door to door assistance and are trained in working with individuals that may require additional assistance.
